How Do I Record Vision?

 

 

This screening is done in the fall. In the Fall it is one of the 45 day requirements.

·      Event Date – Date the screening took place

·      Status - Passed/Complete, Needs Re-screening Failed 1st, or Failed needs Referral

·      Description— Enter the tool you used to screen: Vision Chart or Photo screener

·      Results—L=, R=, Both

·      Agency Worker – Your Name

·      Notes— Timestamp your entry. Document any conversations and follow-up with families
