How Do I Input Data for Hearing?

 

 

This screening is done in the Fall. In the Fall it is one of the 45 day requirements.

            ·      Event Date – The date the screening took place

            ·      StaTus – Passed/Complete, Failed 1st, need Rescreen, Failed 1st, or needs Referral

            ·      Description—Enter the tool you used to screen the child. Either Audiometer or OAE

            ·      Results—L=, R=

            ·      Agency Worker – Your name

            ·      Notes—Timestamp your entry. Document discussions and follow-up with parent.
